We offer a progressive recovery program that allows our team of highly skilled and trained rehabilitation and nursing professionals to assist you in gaining and maintaining your optimal level of independence.

Admission to MeadowBrook of Granby
For the Short Term Rehab Patient, Patients hospitalized for illness, surgery or trauma will be evaluated for our short-term program upon referral from the hospital, the physician, or the family, as soon as discharge from the hospital is anticipated.

Perhaps your first contact with us will be prior to your hospitalization when inquiring about a short-term stay following elective surgery. We will make every effort possible to secure a placement for you, based on your anticipated discharge from the hospital.

Just call for information or a personal tour. Our Admission Coordinator will make every effort to insure that you are well informed and comfortable with the admission process, including what to bring, what to expect from the program, and how long you might expect your stay to be.

Therapeutic Recreation
Whether you are a short-term patient or a long-term resident, our Therapeutic Recreation Department provides a wide variety of leisure and structured recreational activities. An important part of our care plan is giving each resident an opportunity to participate in daily activities. Our program is diverse, innovative, and carefully designed. It is focused toward greater independence and self-fulfillment. Volunteers are an integral part of this team. Some of our many activities include:

Residents receive a calendar of events from which they can choose activities that best suit their individual interests and abilities.

Social Services
Our professional Social Services Department plays an integral role in helping long-term residents adjust and thrive in their new community. Our staff also serves as an advocate of patients and family rights, helping resolve issues concerning illness, placement within the facility, and lifestyle changes. The department also serves as a resource for educational materials on such subjects as heart disease, diabetes, cancer, and Alzheimer's Disease.

For the short-term patient, our social workers coordinate services needed in the community following their discharge. This may include continuing care by a visiting nurse, home health aides, Meals on Wheels, assisted care options, or physical and occupational therapies at home. Our staff helps in every way possible to get our patients to the next step of independence in the community.
