Where We Provide...Rehabilitation Services

We provide individualized physical, occupational and speech therapy. If you need to recuperate from surgery, injury or an illness, The Summit at Plantsville has a team of encouraging professionals who provide the physical and emotional support needed to achieve optimal recovery while preparing for a safe discharge home. Our team conducts on-site home evaluations along with extensive discharge planning. In our state-of-the-art gym, our highly trained therapists work closely to customize a treatment care plan that is specifically designed to maximize the resident's potential.

Dementia Care

Individuals with dementia have special cognitive, psychosocial, recreational and physical needs. Our secure dementia care unit, Meadowview, offers residents individualized programs and services designed to meet the needs of people with Alzheimer's disease and other related dementias. Meadowview offers physical and cognitive activities, specialized entertainment, pet therapy, intergenerational and dementia specific programs.

Admissions

Please take the time to visit our community and meet our dedicated staff. If you or a loved one needs to recuperate after an elective surgery, pre-booking is available. Most major insurance carriers are accepted. Tours are available at any time. However, if you would like to book a private tour, we encourage you to call ahead.
